Yellow terra-cotta panel with female mask, I am unsure of the unusual head-piece depicted, it somewhat resembles a Shriner's hat to me. This came from East 6th Street Ave C and is circa 1905 if that helps for figuring out the hat. The damage on the left and top came during it's removal- the bricks around it were extremely tight and prying on the panelfrom the back side through a hole in the wall loosened it but the fragile edges broke off.
